This Privacy Policy of Dedicated Fitness & Wellness, LLC (the "Practice," "we," "us," or "our") describes how we collect, use, and share personal information when you visit our website at www.dedicatedfitnesswellness.com (the "Site") or otherwise interact with us online, such as by completing a contact form, scheduling a discovery call, signing up for our emails, or communicating with us by email or text (together, the "Services"). Reading this Privacy Policy will help you understand your privacy rights and choices. If you do not agree with our policies and practices, please do not use our Services.
YOUR HEALTH INFORMATION AND OUR NOTICE OF PRIVACY PRACTICES
This Privacy Policy covers information collected through the Site and our general online communications. Once you become a patient, the health information you provide to us or that we create in caring for you (such as intake forms, health history, lab results, visit notes, and prescriptions) is protected health information. We handle protected health information in accordance with the Health Insurance Portability and Accountability Act (HIPAA) and applicable Delaware law, as described in our Notice of Privacy Practices, which is provided to you at enrollment and is available upon request. If this Privacy Policy and our Notice of Privacy Practices conflict regarding your protected health information, the Notice of Privacy Practices controls.
Please do not send detailed health information through the Site's general contact form or by regular email. Patients should use our secure patient portal for health-related communications.
